Bachelor Assignment

The "Mijn Re-integratie Platform" is an online tool designed to inform, connect, and motivate people with chronic conditions and their support networks during re-integration to work. This thesis explored how to design such a platform by combining "Mijn Re-integratieplan," the "My Patient Journey" model, and online group coaching. Research included desk research, workshops, surveys and interviews to define th needs and wishes of stakeholder and essential features. A first concept was created in Figma and tested with stakeholders and their feedback shaped a refined second concept. 

The final outcome is a functional prototype supporting users through clear information and collaboration tools, along with recommendations for expanding and improving the platform. The project delivers a digital tool that better supports people with chronic conditions in returning to work, while highlighting areas for further development.
